Certified Installer Dispute Resolution: Opt-Out Form
To opt-out of the Dispute Resolution Process section of the Andersen Windows, Inc. Certified Installer Limited Warranty, you must do the following within one year of the date of purchase of your Anderen® products:
1. Check the box below.
2. Provide the property owner's name, telephone number, email address and the address where Andersen® products were installed.
3. Date the form.
4. Submit the completed form by clicking the Agree button
A confirmation number will be provided after your request has been received. Please retain this confirmation for your records.
